You will need to bring the following documents:
- Child's birth certificate
- Parent ID
- Proof of Address (electric, water, cable, lease/mortgage)
- Copies of ALL household income (1 month of most recent pay stubs, SSI, SNAP, most recent 1040, Social Security, Child Support, Unemployment, etc.)
Additional Documentation if applicable:
- Documentation of disability: Early Head Start requires a copy of the IFSP. Head Start requires the ESE application ticket. or a copy of the Individual Education Plan (IEP)
- Custody papers if not the natural parent
- Proof of pregnancy is required when applying for Early Head Start
Once the application is complete you will need to apply and obtain a VPK Voucher (4 year old ONLY) through the Early Learning Coalition.
